Pentathlon Registration Forms are now available for all Pentathlon Participants!

You have three ways to complete the registration form:

1. Print off the form, fill it in by hand, scan in the completed form, and email back to the Pentathlon Coordinator at idpentguy@wnysca.org
2. Use a PDF program to fill-in the registration fields, save the completed form, and email to the Pentathlon Coordinator at idpentguy@wnysca.org
3. Complete the form in Adobe and click the “Submit Form”. This feature will automatically send your completed form to our Pentathlon Team. **May not be available for all users**
